A statement under penalty of perjury that you have a good-faith belief the material was removed due to mistake or misidentification

Your name, address, telephone number, and a statement that you consent to the jurisdiction of the federal court in your area (or where TrackAndEarn.com is located)

3. Repeat Infringer Policy
TrackAndEarn.com may terminate user accounts or access privileges for those who are repeat infringers, in accordance with the DMCA and applicable laws.
